We are excited to announce the addition of Chronic Care Management Services for Medicare patients at our practice. Chronic conditions, for example, diabetes, high blood pressure, heart disease, and depression, are ongoing medical problems that must be managed effectively in a partnership between the healthcare team and the patient to maintain the best possible health.
If you are a Medicare patient, you are eligible for this enhanced program. Our nurse, Anna Norwood, BSN, RN, PCCN, will call you once per month to work on your overall health, and care for your needs, whatever they may be. Please ask about chronic care management at your next visit.
